Opportunities

1. Eligibility

- Program is open to individuals age 18 or older.

- Referrers do not need to be business owners or agencies.

- Employees of The Flower Pot may participate only with prior written approval.

- Program applies only to private-pay residents.

- Residents using Medicaid, Medicare, waiver programs, or any government-funded services are strictly excluded from this program.

 

2. Referral Requirements

- A referral must be submitted before the prospective resident tours, applies, or contacts The Flower Pot.

- The referral must include the resident’s full legal name and contact information.

- Only new leads qualify. Returning residents or previously contacted prospects do not qualify.

- Referrer must sign the Referral Bonus Acknowledgment Form before any payment is approved.

 

3. Bonus Structure

- The total referral bonus is $2,500.

- Payment is divided into two parts:

  • $1,000 paid after the resident moves in and completes the admission process.

  • $1,500 paid after the resident remains in good standing for 45 consecutive days.

- Payment is made by check only.

- Referrer must provide valid identification to receive payment.

- Only one referrer will be paid per resident.

 

4. Resident Requirements

A referral qualifies only if the resident:

- Moves into The Flower Pot as a private-pay client.

- Pays all required deposits and move-in costs.

- Remains in good standing for 45 days (for the second payment).

- Is not receiving or applying for Medicaid, Medicare, waiver programs, or other publicly funded long-term care benefits.

5. Legal Compliance

- This program cannot be used for Medicaid or Medicare beneficiaries.

- The Flower Pot follows all anti-kickback, health care, and assisted-living regulations.

- The facility reserves the right to request documentation confirming private-pay status.

- Any violation or attempted misuse disqualifies the referrer immediately.

 

6. Program Changes

- The Flower Pot may modify or discontinue the Program at any time.

- Bonuses for previously approved referrals will still be honored as long as all requirements are met.

 

7. Tax Responsibility

- Referral bonuses may be considered taxable income.

- Referrers are responsible for reporting the income to the IRS.

- A W-9 form may be required before payment is issued.

 

8. General Conditions

- Submitting false information results in automatic disqualification.

- The Flower Pot reserves the right to approve, deny, or withhold bonuses at its sole discretion.

- Participation in the Program does not create employment or partnership status with The Flower Pot.
